Overview

To request medical records, sign a copy of the Authorization for Release Form or the Authorization for Release Form Spanish. This form can be printed from the website, picked up in person, or mailed to you at your request. Return the form with a copy of your current photo ID. You can return the form in person, by mail to the address to the right, or by fax to 785.251.5697.
